We'll send you our new patient intake forms to complete before your visit. Thorough preparation allows Dr. Lee to devote your full appointment time to you.

Enjoy a comprehensive, unhurried appointment with Dr. Lee. Together you'll review your history, discuss your goals, and build your personalized care plan.

New Patient Consultation First visit, comprehensive $650

Return / Follow-Up Visit Established patient $280
Medicare Registration Fee One-time, due at booking $300
Annual Membership — Enrollment Initial enrollment fee $600
Annual Membership — Monthly 11 months (all visits included) $150/mo
Thyroid Ultrasound & Interpretation In-office procedure $150
Biopsy Procedure In-office procedure $350
FibroScan Liver stiffness & fat assessment $150
E-Visit — Simple Asynchronous medical guidance $75

We participate in and bill Medicare only. We do not bill commercial or private insurance. A Superbill is provided at each visit for out-of-network reimbursement. Payment is due at time of service.
